The cargo ship that crashed into the Francis Scott Key Bridge in Baltimore, killing six people in March, lost power twice because of electrical circuit breakers tripping unexpectedly before it slammed into the span.

The findings were revealed in a preliminary report by the National Transportation Safety Board, which said the ship suffered two blackouts before it hit the bridge, leaving it without propulsion to keep it from hitting one of the bridge’s piers, NBC News reported Tuesday.

One of the blackouts occurred about 10 hours before the crash and was triggered by a crew member’s error, the report found.

The captain of the Singapore-flagged container ship told a local pilot that the ship was in good working order. Two tugboats helped guide it out of its berth, and shortly after the pilot turned control over to an apprentice, the first blackout occurred. An emergency generator restored power, and the pilot asked for assistance and ordered the ship anchored. A second power failure occurred, which was also overcome by a generator, but left the ship without propulsion.

The board is still investigating what factors may have contributed to the crash, but said tests of the ship’s fuel found no problems. It is also assisting local authorities in determining whether protections for bridge piers in the harbor need to be improved.

The bodies of the six victims who had been working on the bridge were recovered over several weeks of searching. On Monday, explosive charges brought down a section of the bridge that had fallen onto the ship, freeing it.
