Hospital 'forgets' to notify family of woman's death. "Empty bed"
It happened in Matosinhos. ERS issued an instruction to the health unit so that it does not happen again.

A woman living in the North of the country only found out that her mother, who had been admitted to the emergency department of the Matosinhos Health Unit with "difficulty swallowing and pneumonia", had died when she went to visit her the next day.
The case was reported by the Health Regulatory Authority (ERS), which issued an instruction to the health unit in question.
According to the ERS, the woman made a complaint in which she alleges that, when she went to the Matosinhos Health Unit to see her mother, who had been admitted to the hospital on 6 January, she found "the bed empty".
When she asked about her mother's whereabouts, she was told that she had "died at 2:35 am", several hours before visiting hours.
In response to the complaint from the patient's daughter, the hospital regretted the incident, apologised and assured her that this 'oversight' is not a "habit of the doctors in the Internal Medicine service, nor of the attending physician, and should not have happened".
After a "careful and rigorous" analysis of the case, the ERS concluded that the Matosinhos Health Unit "did not show itself to be in line with the protection of the rights and interests of users", namely "the right of the companion to be duly informed, in a reasonable time, about the patient's situation in the different stages of care, ensuring special care, speed and correctness in the communication of sensitive information, such as the death of a family member".
The ERS thus issued an instruction to the Matosinhos Local Health Unit to ensure, at all times, that companions are duly informed about the patient's situation, as well as to ensure "at all times" that procedures are "correctly" followed and respected by all its employees and/or service providers.
